What Happens After You Refer

Referral Received
You send a short referral online or by phone. We acknowledge it and a named coordinator becomes your point of contact.

Initial Conversation
We call you to discuss needs, risks, timescales and funding, and agree a secure way to share full referral documents.

Care Assessment
We carry out a person centred assessment, usually at home, involving the person and their family where appropriate.

Care Begins
We confirm the care plan and start date with you, introduce carers and keep you updated as care is established.

This short form takes around three minutes. Please do not include the person’s name, date of birth, NHS number, full address or detailed medical information. We will agree a secure way to receive full details after we contact you.
