A Safe, Supported Return Home

After an illness, fall or operation, even familiar tasks can feel harder than before. Getting washed, making a meal or managing new medication may need a helping hand for a while.

Our carers provide practical support in the days and weeks after you come home, so you can rest, recover and rebuild your independence. We work alongside hospital discharge teams, your GP and your family to make the move home as smooth as possible, and we adjust your support as you get stronger.

Hospital discharge support is part of our wider domiciliary care service, so it can continue as ongoing care if you need it.

Your Journey Home

Every recovery is different, but support usually follows a simple path:

  1. Before Discharge

    We speak with you, your family or the discharge team to understand your needs and plan your first visits.

  2. Your First Day Home

    Your carer helps you settle in, checks you have what you need and makes sure you feel safe.

  3. The First Few Weeks

    Regular visits help with personal care, meals and medication while you rest and recover.

  4. Looking Ahead

    As you get stronger we reduce support, or continue ongoing care if that is right for you.

Working With Discharge Teams

We work with hospital discharge coordinators, social workers, GPs and community teams to help people leave hospital safely and on time.
