Peace of Mind With Every Dose

Keeping track of tablets, times and repeat prescriptions can become difficult, especially when there are several medicines to manage. A missed or doubled dose can quickly affect health, confidence and independence.

Our carers bring structure and reassurance to your daily medication routine. We agree exactly what support you need during your assessment, record it in your care plan and follow it on every visit. Families can feel confident that medicines are being taken as prescribed.

Medication support is part of our wider domiciliary care service, so it fits easily alongside personal care, meals and companionship.

Support That Matches Your Needs

We only give the level of help you need, so you stay as independent as possible.

  1. Level 1

    Prompting

A friendly reminder that it is time to take your medicines, which you then take yourself.

  2. Level 2

    Assisting

Practical help such as opening packaging, pouring a drink or passing you your medicines, while you stay in control.

  3. Level 3

    Administering

Where authorised and agreed in your care plan, our trained carers give you your medicines and record each dose.

The level of support is agreed with you, your family and, where needed, your GP or pharmacist, and reviewed if your needs change.

How We Keep Medication Safe

  • Clear Care Plans. Your medicines, doses and times are set out in your care plan so every carer knows exactly what to do.

  • Trained Carers. Carers who support with medication complete medication training and are checked for competence.

  • Accurate Records. Every prompt or dose is recorded on a medication record at each visit.

  • Right Time, Every Time. Visits are planned around your medication times wherever possible.

  • Concerns Acted On. If a dose is refused, missed or causes concern, we follow our procedures and contact your family or GP as agreed.

Working With Your GP, Pharmacist and Family

Good medication support relies on everyone working together. With your permission, we keep in touch with your GP, pharmacist and district nurses so your care plan always reflects your current prescriptions.

When your medicines change, we update your plan and make sure your carers know straight away. We also keep your family informed in the way you choose, so nobody is left wondering.
